Virginia High School League link to athletics physicals
A physical is needed to participate in any school sports. It is a 4 page form that needs to be completed by a parent and a doctor. Insurance is also required and needs to be documented on the last page of the physical.

FCPS link to Emergency Care Cards
This form is needed for each season an student participates in a sport. It is kept on file in case an emergency contact is needed.
