Fill out and Print the new physical form

This needs to be completed and signed each season per athlete.

This document should be signed by athlete and parent. All pages of the document must be turned in together as 1 document to the Activities Office prior to the start of the season.

A physical is needed to participate in any school sports. It is a 4 page form that needs to be completed by a parent and a doctor. Insurance is also required and needs to be documented on the last page of the physical.

This form is needed for each season an student participates in a sport. It is kept on file in case an emergency contact is needed.

All student athlete who need an inhaler need to have an authorization on file.
