ALL FORMS NEEDED PRIOR TO TRY OUTS

Please turn the completed VHSL Physical and Emergency Care forms into the Lee Activities Office. Please contact the Activities office or the Athletic Trainer, Ms. Porter, with any questions.

Virginia Code 22.1-271.5 requires all student-athletes and a parent to complete a concussion education program each year. Please click on the appropriate link below to begin the program. Upon successful completion of this program, the school will have your record updated, this is automatic, you will NOT be required to provide a certificate of completion.
Students and parents must complete separate programs.
Parents need only complete the program one time for all their children participating in FCPS athletics. Please click on the link below to begin the program.
